Is exaltation no longer available for latter day saints who are wayward, fallen off the straight and narrow and die with unresolved sins? Is there any hope for such as one to do a 180 and repent in the post mortal world leading up to the final judgment? Or, is this merely wishful thinking?

Joseph Smith wrote that God's ability to forgive is immeasurable. The difficulty is the spirit who learns to do and want evil, it is much harder to change the spirit without the body than with it. So after one has passed from their mortal existence to the post-mortal, and repent and desire to change, they will eventually be reunited with their flesh that hasn't been with them and learned the rules of being repentant.
Latter-day doctrine teaches it is never too late to begin to repent, and Isaiah of old speaks of sins being as scarlet through Christ can be made white as snow, and a full cleansing can occur.
I feel the Key is Ether 12:27 27 And if men come unto me I will show unto them their weakness. I give unto men weakness that they may be humble; and my grace is sufficient for all men that humble themselves before me; for if they humble themselves before me, and have faith in me, then will I make weak things become strong unto them.
So I would advise beginning now to call upon God and seek this Jesus, who is mighty to save, as All things are possible through Christ.

There is always hope. That's what the atonement is all about. Until the final judgement, there is always time to repent. Granted, once a person has left this world and no longer has a physical body, repentance may become more difficult especially with those sins that involve the mortal body.
In the end, only God can make the judgement. Only he knows the particular challenges and strengths of each of us and the intent of our hearts.
The prophets and apostles have promised that those parents who are sealed in the temples and are true to their covenants and make a sincere effort to teach and guide their children will be able to enjoy their relationship with their children, even if the children choose to go their own way and stray from the path. Sooner or later, perhaps even after this life, they will feel the pull of the everlasting gospel and begin to come back. Even if they do not fully repent and receive their full exhaltation, they will receive the highest degree of glory they can endure and their eternal family relationships will be intact.
Trust the Savior. Don't lose hope. He is very good at what he does.

Absolutely available!! Think of the Savior hanging on the cross, telling the sinner that he would be with him that day in the spirit world. The Savior knows our hearts, but most of all, the gospel is a message of good news, that as we turn to him, he receives us with open arms!!
