I've never heard that one need include the name of an ex-spouse on a baptismal record. I can't imagine why that would be necessary. You might want to double check with the missionaries who are teaching you, but I don't think mentioning your ex will be necessary.
That question is on the baptismal paperwork, but I'm pretty sure that's just record-keeping, similar to the information we put into Ancestry. It's family tree information. But I'm not sure that you HAVE to put that information in. I would check around the questions and answers at LDS.org, but it might be simpler to just ask your bishop. The missionaries might know as well.
